Continuing the Conversation – Multiple & Complex Needs Network in Kirklees

In May 2020 the WY-FI Project will come to an end.

Over the past 5 years we have been developing and delivering personalised care and system change initiatives, to consider how best to respond to the needs of the most entrenched clients around homelessness, addiction, mental health and offending behaviours.

We have now consolidated some of the findings into an evaluation report and would like to take the opportunity to share this with partners and to look at how we can use the findings to shape the future of multiple and complex needs in Kirklees.

The day will contain speakers and workshops and will focus on Person Centred Care, Co-Production, Multi-Agency Partnerships and System / Policy Change.

This is a prime opportunity to link in with a wealth of regional and national data provision and to look at opportunities for embedding multiple and complex needs within existing service provision.

A buffet lunch will be also be provided.
